Employee Time-Off Request

Time-Off Request Policy

Harmony Heights Home Health Services LLC understands that employees may occasionally need time away from work for vacations, appointments, family obligations, personal matters, or other circumstances.

Harmony Heights Home Health Services LLC — Employee Time-Off Request Policy

Employees are expected to submit planned time-off requests at least two (2) weeks in advance whenever possible. Providing adequate notice allows the agency time to review the request, coordinate staffing, and ensure continuity of patient care.

Requesting Time Off

  • Employees must submit a completed Employee Request for Time Off Form to their supervisor or designated office staff.
  • Submitting a time-off request does not mean the request has been approved. Employees must continue to report for all scheduled visits, shifts, and assignments until they receive confirmation from management that their request has been approved.

Approval may be based on:

  • Patient care and staffing needs
  • Availability of qualified replacement staff
  • Previously approved employee requests
  • Amount of advance notice provided
  • Employee’s assigned visits or schedule
  • Agency operational needs

Employees may not independently transfer patient visits, exchange assignments, or arrange replacement coverage with another employee without approval from Harmony Heights Home Health Services LLC.

Emergencies, Illness & Unexpected Absences

  • Harmony Heights understands that emergencies and unexpected illnesses may occur.
  • If an employee is unable to report for a scheduled shift, visit, or patient assignment, the employee must notify their supervisor or the agency office as soon as possible in accordance with the agency’s call-off procedures.
  • Notification to a patient, patient’s family member, or caregiver does not replace the requirement to notify the agency.
  • This is especially important so that Harmony Heights can make appropriate arrangements to prevent interruptions in patient care.

Employee Responsibility

  • Employees are responsible for confirming whether their time-off request has been approved.
  • Unless management has confirmed approval, employees should consider themselves scheduled and are expected to report for their assigned duties.
  • Failure to report for a scheduled shift or patient visit without proper notification or approval may be addressed according to Harmony Heights Home Health Services LLC’s attendance and personnel policies.

Our goal is to accommodate reasonable employee requests whenever possible while maintaining safe, dependable, and continuous care for our patients.
